Release
Every athlete Havoc Media GRP photographs or films completes this form once. Athletes 18 and older sign for themselves. For athletes under 18, a parent or legal guardian signs. You can opt out of public use, or change your choices, at any time by emailing havocmediagrp@gmail.com.
Please read the terms below, then fill out and sign the form at the bottom of this page. In these terms, “the athlete” is the person being photographed, and “you” is the person signing.
01 Permission to photograph
By signing, you give Havoc Media GRP permission to photograph and film the athlete during the session or event listed, including practices, games, warm-ups, sidelines and media day.
At public games and team events, the athlete may appear in the background of photos of other athletes. Havoc Media GRP will not feature the athlete in those photos beyond the uses allowed here.
You confirm that photography is allowed by the school, league or event organizer, or that you will tell Havoc Media GRP if it is not.
02 How photos may be used
By signing, you allow Havoc Media GRP to use photos and video of the athlete on:
- Havoc Media GRP's website and portfolio
- Havoc Media GRP's Instagram and other social accounts
Prefer to keep them private? Check Keep my photos private in the form. Photos then go only to you and your family and are never posted publicly.
Two uses need your separate OK in the form below:
- Team and league sharing: photos may be shared with the athlete's team, coach or league for their own posts.
- Advertising and promotion: photos may be used in paid ads, flyers or promotional material for Havoc Media GRP.
Havoc Media GRP will never sell the athlete's photos to a third party, or license them to one, without separate written permission.
03 Privacy
- Names: public posts use the athlete's first name and jersey number at most, unless the form allows a full name.
- Schools and locations: public posts will not name the athlete's school, home city or schedule unless the form allows it.
- No tagging of the athlete's personal social accounts without permission.
- Removal on request: if you ask, Havoc Media GRP will take down any public photo of the athlete within 7 days. Removal from posts other people have already shared may not be possible.
- Your contact details are kept private and used only to deliver photos and contact you about the athlete's sessions.
04 Ownership and delivery
Havoc Media GRP owns the copyright to all photos and video. You receive a personal-use license: you may download, print and post your photos on your own personal social accounts, with credit to @havocmediagrp appreciated. Selling the photos or using them commercially requires written permission.
Edited photos are delivered through a private online gallery, normally within 48 hours of the session. Unedited files are not provided.
05 Your signature
By typing your full name and submitting the form, you confirm that you are the athlete and 18 or older, or that you are the athlete's parent or legal guardian; that you have read these terms; and that you agree to them and to the choices you made in the form. Your typed name counts as your signature.
This release stays in effect until you withdraw it in writing by email. Withdrawal applies to future use and does not recall photos already delivered or printed.
